There are two type of fatty changes are seen ….
i)
Fatty degeneration
ii)
Fatty infiltration .
In fatty degeneration and fatty infiltration , there will be
abnormal accumulation of fat in the cell and in both condition , fat is brought
to the cell by plasma from fat depot . According to virchow , in fatty
degeneration , the invisible fat which is normally present in the cell becomes
visible as it is united with cell protoplasm due to disease condition and the
protein of the cytoplasm of the cell will convert into fat . This way of
degeneration I termed as fat phanerosis . here , percentage of fat will
increase . But recently it is seen that protein will not onvert into fat and
the fatty degeneration will occur due to
the infiltration of fat from , the source of fat when the ell are sick . The
sources of the fat are ------
i)
The depot fat – Thisfat exists in visible form and
present in omentum , subcutaneous tissue and other places of the body . It is
present in the saturated form so that iodine value is low and is not readily
available for use .
ii)
The tissue fat – This fat exists in invisible form and
is the essential component of the cell .but is drawn from depot fat or fat in
blood or from carbohydrate . The depot fat and food fat are neutral and fatty
acids are in an inactive saturated form . In the liver , it will form lipids
like cholesterol , lecithin etc
In fatty degeneration
, there will be abnormal accumulation of stainable dfat in the cell
where the cells are previously uInjured lile sick . The fat is brought to the cell by plasma from fat depot . Here ,
the fat droplet will be smaller in size . Liver , heart and kidney are mostly
affected . Anoxixc condition to the cell and poisoning are the cause of
degeneration .
In fat infiltration ,
there will be abnormal accumulation of fat as a large droplets in the cell is
not previously injured like healthy . There will be systemic metabolic
degeneration like endocrinal disorder or pseudo-muscular dystrophy or excessive fat intake or less utilization
of fat . Here , fat droplets will come to the cell and fusion of fat droplets
will take place , as a result , there will be appearance of large fat droplets
. Fatty metamorphosis is applied where the cells will be accumulated by the fat due to intensive administration of
fat in the diet and this may particularly happens in heart and kidney .
